What Is PSMA PET Imaging for Prostate Cancer?
Prostate Specific Membrane Antigen (PSMA) PET Imaging is an FDA-approved injection for men with prostate cancer. Patients who have an elevated PSA level and have undergone a prostate biopsy that is positive for prostate cancer or had treatment in the past for prostate cancer are candidates for PSMA prostate imaging. This injection aids in detecting cancer in those who are just starting treatment or suspect cancer has come back based on elevated PSA levels. This is not a screening exam for undiagnosed prostate cancer.
How Does PSMA Testing Work?
A PSMA Tracer is administered to the patient through an intravenous injection one hour before the imaging session takes place. It is a radioactive imaging tracer that binds to prostate cancer cells to aid in pinpointing prostate cancer lesions in the prostate gland as well as outside of the prostate gland (metastatic lesions) to assist in a more complete and successful treatment plan for the patient.
What Are the Benefits of PSMA PET Imaging?
In comparison with other prostate cancer detection techniques, Prostate Specific Membrane Antigen (PSMA) PET Imaging offers many benefits for men with prostate cancer.
- It enables more accurate detection of prostate cancer, facilitating improved treatment planning and targeted care.
- It is highly effective in locating tumors not only within the prostate gland but also
- throughout the body if the cancer has spread.
- No fasting is required prior to the scan.
Who Should Consider PSMA PET?
PSMA Imaging Scans may be right for you if you are a man who has been recently diagnosed by biopsy with prostate cancer and/or:
- is just starting the decision-making process for your prostate cancer treatment.
- suspect your prostate cancer has come back based on elevated PSA levels.
What to Expect During a PSMA PET Scan
Prior to Your Appointment
- Drink ample amounts of water the day of the exam.
- Please dress comfortably. If you have any metal items when you arrive, you will be asked to change into a gown or remove the metal before the exam.

During the Scan
- You will be asked to lie on a padded table that moves into the scanner. The Technologist will maintain visual and auditory contact with you throughout the scan.
- It’s important to remain still during the scan, as movement can impact the accuracy of the results.
- The scan itself will last about 15-20 minutes.
- The entire exam from check-in to check-out typically takes around 2.5 hours to complete
After the Scan
- Most patients can resume their regular activities right away.
- Since a tiny amount of the tracer lingers in your body, it’s important to drink a lot of water to assist in flushing it out of your system.
Are There Risks Associated with PSMA PET Imaging?
This FDA-approved imaging method is PET scans is generally safe, but with all PET scans come with potential risks to consider:
- Fluorine18-PSMA injections, adverse reactions reported in ≥0.4% of patients in clinical studies were diarrhea, blood pressure increase and injection site pain.
- Radiation exposure: Although the radiation dose is low, cumulative exposure from PET scans and other imaging tests may be a concern.
- Allergic reactions: Rare but possible, individuals may experience allergic reactions to the radioactive tracer used.
- Pre-existing medical conditions: Conditions like kidney or liver disease may impact tracer clearance, requiring special precautions.
